Most of the time it's real.You can say 90–95 % of the time.Nowadays actors and actress are becoming more and more professional. They do not hesitate in lip locking on screen , it's quite normal for them as they are just doing their job. They are mentally prepared for it.

Although even today some of the actors/ actress do not approve kissing on screen. Celebs like Salman Khan , Sonakshi Sinha , Shilpa Shetty , Riteish Deshmukh , Asin , Sunil Shetty and Tamana Bhatia never said yes to on screen kissing.

Also there are celebs who broke their no kissing rule. Among them prominent are Shah Rukh Khan ( kissed Katrina Kaif in Jab Tak Hai Jaan ) and Ajay Devgan ( kissed Erika Kar in Shivaay ).

Now VFX technology is also used in filming a kissing scene, for example Kajal Aggarwal and her co-star Suriya kissing scene in film Maattrran.
